Community Health Centers of Greater Dayton - West Carrollton Health Center Construction
The project will construct a new, 11,200 SF full service community health center in West Carrollton, providing medical, dental and pharmacy services for low and moderate income residents of West Carrollton and the surrounding communities in Montgomery County. The $3.7 M project will redevelop a vacant site at 630 E. Dixie Dr. The site is owned by Community Health Centers of Greater Dayton, and the requested funding is needed for construction.

Community Details
The project will construct a community health center and provide medical, dental and pharmacy services for low and moderate income residents that are currently underserved in a new state-of-the-art facility. There are 15,800 low and moderate income individuals who reside in the 45449 and 45342 zip code areas of which over 13,000 are not served by a community health center and do not have a medical "home". The center will significantly increase access to healthcare for those individuals. The center will have a total staff of 27 including physicians, nurse practitioners, dental and pharmacy in a single location to best serve low and moderate income individuals.
